Are your service records clear enough to support the work delivered?

ProviderProof helps Australian Support at Home providers identify weak documentation, unclear evidence and claim-readiness risks before they become recurring operational problems.

We independently review up to 10 de-identified documentation samples and provide a practical PDF report showing what is documented well, where the evidence is weak or unclear, which issues appear repeatedly and what your team should improve first.

Problems we identify

Where documentation often becomes unclear or difficult to rely on

ProviderProof reviews submitted samples for common documentation weaknesses that can make it harder to understand what was delivered, why it was required and how the recorded service is supported.

Unclear service evidence

Notes that confirm a visit occurred but do not clearly explain the support delivered, the staff member’s actions or how the service period was used.

Vague or generic wording

Records that rely on phrases such as “usual support provided”, “all good” or other wording that does not show what actually occurred.

Weak outcomes and follow-up

Documentation that does not clearly record the participant’s response, what was completed, concerns identified or any required follow-up.

We also look for:

We also look for:

• Weak connection to participant needs, goals or reason for support

• Care management notes that do not clearly show planning, coordination or review

• Contradictory, ambiguous or unsupported wording

• Repeated staff, template or internal review gaps

• Notes that may not clearly support the recorded service if questioned

de-identified documentation samples

What we review

Each applicable sample is assessed for service evidence, specificity, participant relevance, outcome clarity, time and task clarity, claim-readiness, risk language and quality alignment.

Care management evidence

Where care management documentation is submitted, we also review whether the record clearly shows relevant planning, coordination, consultation, review or follow-up.

FAQ

Frequently asked questions

Is ProviderProof a compliance auditor?

No. ProviderProof is a documentation quality and claim-readiness review service. The Mini Audit does not certify compliance, replace an official audit or determine whether your organisation meets every applicable requirement.

Does the Mini Audit guarantee that our claims will be approved?

No. ProviderProof identifies documentation strengths, weaknesses and areas where evidence may be unclear. It does not approve claims or guarantee payment.

Do we need to attend a call?

No. Checkout, document submission, communication and report delivery can all be completed online or by email.

What documents can we submit?

You may submit up to 10 de-identified examples, including care notes, visit notes, service records, care management notes, de-identified claim examples, internal documentation samples or staff templates.

Should we submit only our strongest notes?

Not necessarily. A realistic mixture of typical, stronger and weaker records can provide a more useful view of recurring documentation patterns.

Can we submit identifiable participant records?

No. All records must be properly de-identified before upload. Do not submit participant names, residential addresses, dates of birth, Medicare numbers, health identifiers, full contact details or unnecessary sensitive personal information.

Will the report include rewritten notes?

Where useful, the report may include examples of stronger documentation wording. These examples will not invent actions, participant responses, outcomes or other facts that were not documented.

What happens after we receive the report?

You can use the report to prioritise staff guidance, strengthen templates and improve internal documentation reviews. Questions about the findings can be handled by email.
