Does the Mini Audit guarantee that our claims will be approved?
No. ProviderProof identifies documentation strengths, weaknesses and areas where evidence may be unclear. It does not approve claims or guarantee payment.
Do we need to attend a call?
No. Checkout, document submission, communication and report delivery can all be completed online or by email.
What documents can we submit?
You may submit up to 10 de-identified examples, including care notes, visit notes, service records, care management notes, de-identified claim examples, internal documentation samples or staff templates.
Should we submit only our strongest notes?
Not necessarily. A realistic mixture of typical, stronger and weaker records can provide a more useful view of recurring documentation patterns.
Can we submit identifiable participant records?
No. All records must be properly de-identified before upload. Do not submit participant names, residential addresses, dates of birth, Medicare numbers, health identifiers, full contact details or unnecessary sensitive personal information.
Will the report include rewritten notes?
Where useful, the report may include examples of stronger documentation wording. These examples will not invent actions, participant responses, outcomes or other facts that were not documented.
What happens after we receive the report?
You can use the report to prioritise staff guidance, strengthen templates and improve internal documentation reviews. Questions about the findings can be handled by email.
